ADD / ADHD Evaluations (Attention Deficit Disorder / Attention Deficit Hyperactivity Disorder)

Identifying ADD & ADHD Behavioral Issues

We ask patients for information from a number of different sources when evaluating for Attention Deficit Disorder:

  1. A simple one-page letter from the present teacher, or the teacher who knows the child best, regarding the issues that are prompting the evaluation;
  2. A copy of this year’s and the previous year’s entire report cards (not just the grades);
  3. The Parent and Teacher Behavior Rating Scales: the Connors assessment (for Dr. Glines) or Vanderbilt assessment (for Dr. Cross and Dr. Vedova).
